Health Care Power of Attorney

 

This document is similar to the Living Will, but it is not an “end of life” document. The expectation with the Health Care Power of Attorney is that you are going to recover and will be able to make your own decisions again. The purpose of this document is to inform health care providers of your wishes if you are temporarily unable to speak for yourself, or are unable to appreciate the information your caregivers are trying to convey to you. For instance, if you are in a car accident and are unconscious, your Agent is authorized to make medical decisions for you. You are asking your Agent to step into your shoes and make decisions you would normally make for yourself if you were able to do so. Some examples of the decisions your Agent may make include treatment options, admission to and release from the hospital, as well as whether or not you will undergo surgery. This list does not include all of the decisions your Agent will have to make, which makes it very important to have a frank conversation with your Agent about what types of decisions you would like him or her to make before decisions need to be made on your behalf.
